Hearing Request
This form is to be used if you are contesting a Parking Ticket. Please complete the statement and completely fill in the required information on the bottom portion of the form. You can then mail the form with the parking ticket to the address on the top of the hearing request form. |

Appeal of Hearing Officer Decision
This form is to be used for appealing a decision on a Parking Ticket already rendered by the hearing examiner. Once you complete the form you may file for the appeal in person at the Cleveland Municipal Clerk of Courts Office, Civil Division 2nd Floor, Monday through Friday 8:00 a.m. to 4:00 p.m. The cost for filing is $61.00 which must be paid in cash or certified check. |

Motion to Vacate
This form is a Legal Document and should be used and completed only when a ticket is in judgment. Tickets that are 90 days and older with two late penalties added are usually in judgment status. This form must be presented in person to be filed and time stamped by our cashiers at the City of Cleveland Vehicle Impound & Clerk of Courts Payment Center 3040 Quigley Rd. Cleveland, Ohio 44113. |

Transfer of Liability (leasing and rental companies only)
For use by Rental and Leasing Companies for parking tickets only. This form is used to name (nominate) the driver who had control of the vehicle at the time and date the parking ticket was issued. |
